I hereby consent to the use and storage of my or family’s personal details as outlined above both in paper and electronic format for the purpose of club management. I confirm that I am authorised to share this information for any family member listed and that I am a parent or legal guardian(s) of any minors on this form.
The data will be used to assess the membership category, for the club to contact you in relation to club events and membership administration. The data will be stored on this paper at the clubhouse and at the home of the membership administrator or treasurer of the club and it will be stored in a spreadsheet available to committee members. Occasionally other club members may be given your details (name, phone number, email address, membership category) to contact you for club events. Third parties will never be given access to your personal data, without further specific consent. At all times you can request a copy of the data we hold, and at all times you can request r the deletion of your data. Deletion of your data however means immediate cessation of your membership. The data will be stored for the duration of your (continued) membership with the club plus 1 year. After that period the club may contact you to maintain a database of former members, however you will be asked again for consent to store your data.
I am aware that Kells Tennis Club cannot be held accountable for any personal injuries or theft or damage to property occurring when using the club’s facilities
